Please fax, mail, or drop off registration forms to the office you will be seen at.

 

       MAKING APPOINTMENTS

Patients are seen at Jupiter by appointments only, Monday- Friday. Our normal office hours are:

 Monday      9:00-5:00
 Tuesday      8:00-4:00
 Wednsday   9:00-5:00 
 Thursday    9:00-5:00
 Friday         8:00-4:00   
 Saturday      emergency

Patients are seen at Pahokee Monday- Friday. Our normal office hours are:

**EKG machines are on site once per monthly for monitoring those on certain medications, please see the receptionist for the date**

The receptionist is available daily to help you.

 

We try our best to stay on schedule, however, emergencies sometimes arise. If we are seriously delayed, we will try to notify you beforehand.

Occasionally, we may call patients in the waiting room out of turn if they are urgent exams.

We ask your patience if you have to wait. We know your time is valuable and we hate to keep you waiting!

    

 

  

 YOUR APPOINTMENT:

   On your first visit we will create a

permanent record. We will ask you to complete a brief history and registration form. Please bring your insurance card, along with a copy of your child’s shot record. We will have you sign a release to obtain previous medical records.

   Please try to be here at least 15 minutes prior to your first appointment to complete the necessary forms.

 

CANCELLING:

   If you are unable to keep your appointment please call the office as soon as possible. This courtesy allows us to be of service to other patients.

·        If you are more than 10 minutes late we may have to reschedule you.

·        Insurance that require a primary physician must either show our name, or must be able to be verified.

·        If we cannot verify ourselves as the PCP , you will be responsible for the visit.

·        If you have 3 concurrent missed appointments we reserve the right to dismiss you from our practice.

·        We also may charge you a $25  no call no show fee.

 

 

 

 

EMERGENCY CARE:

   If you have an urgent problem during normal business hours, please call the office and speak with the nurse.

For after-hour urgent problems please call the office number and follow the instructions to have the on-call provider paged.

   In a true emergency go directly to the hospital emergency room.

 

TELEPHONE CALLS:

    To save you time and the expense of follow-up visits, once treatment has begun, we ask that you complete all recommended courses of treatment.

   Please call my office if you have any questions regarding your child’s condition, medication, or treatment. My staff is specifically trained to answer your questions. If you need to speak to a provider you will need to inform the staff of the reason for your call so that we may be fully prepared to answer your questions. We cannot return calls where the nature of the call is not known.

   If you need a medication refill you will need to provide my staff with the medication name, and the pharmacies telephone number. We ask that you call at least 2 days in advance.

Many illnesses have similar symptoms, so if your child has developed complications or new problems, please understand there may be a need for re-examination.

 

 

 

 

CONSULTATIONS:

   Occasionally we can deliver better care by seeking the opinion of other physicians. Thus, we may suggest a referral to a specialist.

Certain insurance require prior approval before we can send you to another provider. The staff will inform you as to the procedures that apply to your specific insurance. Also be aware that some referrals take up to 14 days.

 

INSURANCES:

   We accept most private insurance, Medicaid, and Florida Healthy Kids.

If you have an HMO we MUST be the PCP listed on the card.

   You MUST bring your card with you at every visit. Insurance plans may cover anywhere from 0 to 90% of your medical costs. You are responsible for any amount not covered by your plan.

   Our billing company will submit to any company we are providers for. If we are not a provider for your plan, you will be responsible at the time of visit, and we will give you a copy of the paid bill.

   If you have any concerns or complaints, you will need to call your insurance companies claims department.

·        Copays are due at time of visit

·        Payment must be rendered at time of visit( unless we are a provider with that plan)

·        You must show us a copy of new insurance as soon as you receive it.
